How quickly will someone visit?

No fixed visit or response time is promised on this website. Timing depends on location, clinical needs, staffing, and scheduling availability.

Can I visit the Chicago office without an appointment?

Office visits are by appointment. Call 773-302-7237 before traveling to arrange a visit and confirm that staff and the appropriate clinician are available.

How do I know which clinician is visiting?

Ask staff for the scheduled clinician’s name, compare it with the Physicians page, and ask the visitor for professional identification. Call the office before care begins if anything does not match.

Which languages are available?

English, Russian, Polish, and Korean support is available. Confirm the needed language when scheduling because availability may depend on the assigned team member.

Do you accept my insurance?

Review the current insurance list, then call to verify your exact plan, network, eligibility, benefits, and any authorization. A plan name on the website is not a guarantee of payment.

Can I send medical information through the website form?

No. The public form is only for contact and routing details. Do not include diagnoses, symptoms, dates of birth, insurance information, or records.

How should a professional referral be sent?
